PBT, a wholly owned subsidiary of ISMIE Mutual Insurance Company, is a
unique entity formed by the Illinois State and Chicago Medical Societies.
Created to provide ‘the finest health insurance and services to the individuals
who have made health care their profession and understand its importance,’ the
health plans are truly designed for physicians, by physicians.
How the plan can benefit you
Consociate Care and PBT have developed a plan offering available to
provider members of the Consociate Care Network, their eligible employees and
family members. Key benefits of the PBT programs include:

pre-certification or second-opinion requirements - except for Organ
Transplants

NO
minimum group participation requirements – PBT will insure all, or just a
portion of, your group practice.

Indemnity, PPO, and HSA qualified High Deductible Health Plans.

Ability to offer up to three (3) different deductible options to your
employees.

Coverage for physicians is completely portable
If you elect coverage in a PBT Health Plan between
June 1 and September 30, 2006:

Every applying participant will be ENROLLED and GUARANTEED
COVERAGE,
and

COVERAGE WILL BE ISSUED AT PBT STANDARD RATES with no
“rate up” due to participant health history.
